Vaccination is often viewed strictly as a childhood checklist, but immunity for certain critical diseases can wane, or doses might have been completely missed during early years. One of the most significant examples is Hepatitis B. While it is a standard part of pediatric immunization today, many adults who grew up before it was routinely offered remain completely unprotected against this serious viral infection.

Determining whether you need an adult catch-up dose of the Hepatitis B vaccine is an important part of long-term preventive healthcare. This is a detailed guide on how to decode this hidden vulnerability and what adults in India need to know.

What is Hepatitis B?

The highly contagious hepatitis B virus attacks the liver. Unlike the common cold, the Hepatitis B virus (HBV) can cause a long-term illness that lasts a lifetime. Chronic hepatitis B is a leading cause of serious liver disease, such as cirrhosis (scarring of the liver) and liver cancer.

It is transmitted via exposure to infectious body fluids like shared medical equipment, sexual contact or even minor household exposures such as sharing razors or toothbrushes with an infected individual.

Why Might Adults Need this Vaccine Now?

The National Immunisation Schedule of India currently recommends starting Hepatitis B vaccination at birth (within 24 hours) and completing it with doses at 6 and 14 weeks. But this widespread, systematic vaccination of infants is a relatively recent public health development.

Many adults, especially those born before the Hepatitis B vaccine was routinely given to infants, never got these early doses. This leaves many adults susceptible to infection and no immunity.

Who Should Consider Catch-Up Vaccination?

Adults who never got their childhood vaccines should definitely talk to a healthcare provider about a catch-up schedule. Indian medical guidelines recommend even greater importance of vaccination for adults at higher risk of exposure. This comprises,

  • Household Contacts: Individuals who reside in the same household as a person with chronic HBV infection.

  • Healthcare Professionals: Doctors, nurses, dental staff and hospital workers who may be exposed to blood at work.

  • Patients with Special Medical Needs: People who are on haemodialysis or who receive regular transfusions.

  • Possible Sexual Exposure: Persons presenting to sexually transmitted disease clinics or sex partners of an HBsAg-positive individual.

Adult Catch-Up Vaccination Schedule:

The standard Hepatitis B catch-up schedule for adults who need immunization is usually a three-dose series. This is typically administered as an initial dose, followed by a second dose one month later, and a final third dose six months after the initial dose (a 0, 1, 6-month schedule).

If the schedule is interrupted after the first dose, the second should be administered as soon as possible, ensuring at least an 8-week interval between the second and third doses. For most healthy adults with normal immune status who complete the primary series, a booster dose is generally not required.

Protecting Your Liver Health!

Hepatitis B is a disease that can be prevented. The best way to prevent long-term liver damage is to be proactive and ensure that you are immunised against the hepatitis B virus.

Your childhood records do not tell the complete story of you. Visit the medical professionals at INVC to review your vaccination history, determine if you need a Hepatitis B catch-up dose, and arrange your adult vaccines at a convenient time.

FAQs About Adult Hepatitis B Vaccination

1. Do healthy adults need a Hepatitis B booster shot?

For most immunocompetent adults who completed a vaccination series in childhood, a booster dose is generally not needed. However, some high-risk individuals or the immunocompromised may need titers drawn and possibly revaccinated.

2. What if I started the adult Hepatitis B series and missed a dose?

If you miss a dose, you don’t have to restart the whole series. If the second dose is delayed, it should be given as soon as possible and the third dose should be given at least 8 weeks after the second dose. If only the third dose is delayed, it should be given as early as possible.

3. Should an adult have a blood test before being vaccinated against Hepatitis B?

Overall, routine screening prior to vaccination is not found to be cost effective for the general population in India. A healthcare provider can discuss your specific risk factors and whether testing is recommended for you.

4. How long does the Hepatitis B vaccine last?

After finishing the entire adult series, protection usually lasts for decades. Most people are protected for at least 20 to 30 years, and many are protected for life, studies suggest.

5. Is the adult dose the same as the dose for kids?

No, the dose depends on the age. In India adult dose is (20 years and above) - 20 mcg (1.0 ml suspension), paediatric dose is (neonates through adolescents up to 19 years old) - 10 mcg (0.5 ml suspension).
